Please remove my address from survey results page. I do not agreed for such use of my personal data! In fact you just destroyed my customer satisfaction, and I will not trust you and will never help in your Customer Satisfaction Developments again.

Remove my email from the results page of your survey page! I wouldn't have given it to you if I'd known you were going to publish it. I wonder how many spam bots will pick it up now? VERY poor design.
I don't even trust the statement on this form now: "Allows other people to email you through our web site. Your address is never revealed." REALLY?
